Welcome to Brokers Choice of American's Life Insurance Illustration Online Request Site.  This site was created to allow you to submit your term, and permanent life insurance illustration requests to us without having to call our office, allowing you to submit a request from your desk, laptop/tablet or phone.  Upon completion of a form, your request will be email immediately into an email account, which will allow us to prepare an illustrations for you.  

There are separate forms depending upon the type of life insurance you are presenting to your client.  The "Blue" tab is for term life insurance quotes.  The "Yellow" tab is for non-qualified Index Universal Life or Universal Life illustrations (you can also request a whole life illustration). The "Green" tab is for our proprietary Qualified Plan Conversion and Tax Strategy or ("QPCTS") report.  

Please make sure that all data fields have data in them.  If the data field is not applicable please enter N/A in the field.  Once all data has been enter please click on the "Submit" button at the bottom of the form.  If the form has been successfully submitted your screen will become blank.  If you want to confirm receipt of your form please send a separate email requesting confirmation to bgastl@brokerschoice.com.
